Pulmonary spot impedance was measured in 74 cases of open-heart surgery and in 26 normal healthy subjects from January 1980 to May 1982. The results and conclusions are as follows : 1. Pulmonary spot impedance of the right upper lung was found to be more efficient for detecting the change of the lung water than transthoracic impedance using tape electrodes. 2. Good correlation (r=0.6〜0.9) was observed between pulmonary spot impedance(Z_0) and obesity (Broca's index). The pulmonary spot impedance(Z_0) was thought to be influenced by subcutaneous fat and other individual factors. Therefore the postoperative change of the pulmonary spot impedance(Z_0) was expressed as per cent of the preoperative values. 3. Pulmonary spot impedance(Z_0) was decreased immediately after open heart surgery in all of the patients studied. Recovery was gradual. Z_0 values of 48 hours after surgery were 73% of the preoperative value in mitral group, and 86% in the remainders. 4. Good correlation was observed between the pulmonary spot impedance (Z_0) and the mean pulmonary capillary pressure (r=-0.54) and Qs/Qt (r=-0.42). Postoperative change in pulmonary spot impedance (Z_0) below 70% of the preoperative value was usually associated with mean pulmonary capillary pressure over 20 mmHg and Qs/Qt over 20%, warning a prompt and adequate treatment with cardiotonic drugs and diuretics.
