Three patients with osteomalacia presumably related to inadequate vitamin D action, two after gastrectromy and one with hypophosphatemia complained of muscular weakness. In order to elucidate the mechanism of muscular weakness in vitamin D deficiency, phospholipid profile and fatty acid components of the sarcoplasmic reticulum (SR) and sarcolemma (SL) were measured in vitamin D deficient and vitamin D replete chicks. The serum Ca levels of the D-deficient chicks were lower than those of the D-replete group. The content and the constituent of phospholipid in SR and SL did not reveal a significant difference between the two groups. Gaschromatographic analysis of the fatty acids in SR demonstrated that C18 : 1 was increased significantly in the D-deficient group. On the other hand, in SL no obvious difference was found between the two groups, but the ratio of C16 : 1 to C16 : 0 was significantly increased in the D-deficient group. The ratio of unsaturated fatty acid to saturated fatty acid (C18 : 1 vs C18 : 0, and C16 : 1 +C18; 1 vs C16 : 0+C18 : 0 respectively) was elevated in the PE of SL in the D-replete group. These studies suggested that the alteration in the fatty acids of SR and SL might be involved in the mechanism of the muscular weakness in patients with vitamin D deficiency and related disorders.
