N-acetyl β-D-glucosaminidase (NAG) and α-L-fucosidase were determined in plasma from 21 children with insulin-dependent diabetes, in age from 5 to 18 years old. Results have shown significantly higher-levels of plasma NAG in diabetes (533.6 ± 127.9 units/ml) compared to age-matched controls (405±77.2 units/ml), and also significantly higher levels of plasma α-L-fucosidase in diabetes (708.5±149.5 units/ml) compared to age-matched controls (541.4±130.7 units/ml). In deabetics, there was correlation between the plasma NAG activity and HbA_1 (r=0.7974), and between the plasma α-L-fucosidase and HbA_1 (r=0.6838). These two enzyme activities in plasma were higher in poor controll-group than in good controll-group. The extremely high level of plasma NAG (1728 units/ml) and α-L-fucosidase (1367 units/ml) in the newly diagnosed diabetics with severe ketoacidosis, decreased to normal level at 24 hrs. after insulin infusion. The isoelectric focusing analysis of plasma NAG activity showed that NAG was consisted of two peaks (A form, B form) and the A form increased in diabetic state. On the other hand, extremely increased A form at onset decreased to normal level with insulin infusion. These results suggest that plasma NAG and α-L-fucosidase activies reflect the diabetic state.
