The clinical effects on the theophylline clearance of long term theophylline administration and asthma attack itself were determined among 16 asthmatic children. (mean age 11.6 yr.) Aminophylline was administered intravenously by constant rate untill steady state level was obtained. Theophylline clearance was calculated from steady state serum theophylline value and infusion rate. 1) 7 severe asthmatics who had been administered theophylline by Round the Clock Therapy for more than 6 months (mean 15.9 mon.) were studied. Their theophylline clearance values showed 0.051 ± 0.007 L/kg/hr and no significant change was recognized comparing with 5 asthmatic controllers (0.053 ± 0.003 L/kg/hr). 2) 5 children with chronic asthma were studied their theophylline clearance during their attack. The result was 0.053 ± 0.009 L/kg/hr and was not different significantly from that of 5 controllers who were studied when they had no attack. 3) These results suggest that any side effects due to theophylline accumulation will not occur and little consideration is nescessary as for the theophylline dosage after long term theophylline therapy or during asthma attack.
