A case of atopic myelitis with increased cerebrospinal fluid (CSF) IgE is reported. A 28-year-old man, who had suffered from atopic dermatitis since the age of 6 years, noted paresthesia of his right upper and lower limbs on November 13, 1998. His serum and CSF IgE levels were high, and mite-specific IgE antibody was positive. His upper cervical spinal cord appeared swollen on the MRI, and T2-weighted imaging detected a high intensity lesion. This is the first case with high IgE levels in both blood and CSF, suggesting atopic myelitis proposed by Kira et al.
