Analysis of Muscle Activity in REM Sleep Behavior Disorder by Means of Polysomnography

Stage 1-REM sleep with a tonic EMG is seen in normal elderly people without physical and mental abnormalities and with no apparent brain stem lesions. Such cases are called idiopathic REM Behavior Disorder (RBD). Six patients with RBD between the ages of 57 and 77 years were examined on all-night polysomnography. Our study of six RBD cases found; 1) no abnormalities in sleep architecture, 2) no distinct abnormalities in the mechanism of expression of REM sleep, 3) abnormalities in motor activities during REM sleep which included a) abnormality in the atonic state and b) an increase in phasic motor activity associated with burst REMs, and 4) an appearance of abnormal behaviors during REM sleep ranging from twitch movements to relatively large body movements. Even though no detectable lesions in the brainstem were found, there is a possibility that some changes in the upper brain which cannot be detected by current techniques may cause RBD. Further studies are necessary to investigate the neuropathology of patients currently categorized as having idiopathic RED.
