The Clinical Results and the Evaluation of Minimally Invasive Direct Coronary Artery Bypass Surgery as a Revascularization for the Left Anterior Descending Coronary Artery via Left Anterior Short Thoracotomy

One hundred and nineteen patients underwent "minimally invasive direct coronary artery bypass surgery (MID-CABG)" as a revascularization for the left anterior descending coronary artery (LAD) since September, 1996 until December, 1999. Eighty-two patients had significant lesions of other coronary arteries at the time of the surgery. Forty-six patients underwent angioplasty for those lesions after the surgery and 27 had undergone before the surgery, however, the prognosis of the patients with those other lesions treated by percutaneous transluminal coronary angioplasty (angioplasty) was unknown. The procedures for all the patients were performed without using any circulatory supportive device. Three patients died after the surgery however, the course was not related to cardiac event. One hundred and one patients underwent postoperative angiogram in early postoperative period, to elucidate the effect of the bypass grafts. As the results, occlusion or stenosis of the grafts were seen in 16 patients, including acute occlusion of the graft in 4 patients. Seven patients underwent angioplasty to the grafts subsequently. Angiogram was performed in long-term period and 27 patients showed excellent patency more than 2 years after MID-CABG. Those results suggested MID-CABG could be obviously regarded as a quite beneficial and reasonable method of revascularization for a certain conditioned patient with lesions of LAD; however, profound considerations would be required for the patients with multi-vessel disease.
