Re-evaluation of Uvulopalatopharyngoplasty (UPPP) for Obstructive Sleep Apnea Syndrome from the Viewpoint of Mesopharyngometry

To clarify the usefulness of UPPP, the oropharyngeal space were measured pre- and post-operatively. It was found that UPPP fulfilled its main purpose to enlarge the oropharyngeal space. The success rate of UPPP was 78%. The content rate for the patient and the surgeon was 87% and 72%, respectively. It was thus concluded that UPPP is a useful operation for obstructive sleep apnea syndrome patients. The possible reason of the low success rate of UPPP was discussed. UPPP alone cannot cure obstructive apnea caused by multiple upper airway obstruction. One should consider the role of UPPP as a part of total anatomical correction of the upper airway. One should also carefully consider the indication of UPPP for the mixed type or central type apnea. Several problems regarding the usefulness of the conventional sleep study were also discussed.
