Laser Scanning Confocal Microscopic Observation of Skin Samples Prepared by Immunofluorescence

In epifluorescent microscopy, fluorescent flare and background glow limit contrast and resolution. This method is thus not adequate for examining fine structures. In this study, to deal with this situation, use was made of confocal laser-scanning microscopy (CLSM), an innovative microscopic system that provides remarkably high resolution. Skin samples stained with immunofluorescence after being obtained from patients with several skin diseases were observed by CLSM. This technique showed clear images of granular, fibrillar and linear IgA deposits in the skin specimens of dermatitis herpetiformis and linear IgA bullous dermatosis, suggesting a clear distinction among the three types of IgA deposition. Confocal imaging also showed intercellular IgG-autoantibody deposits in the normal human skin which was in prior incubated with pemphigus sera, and numerous Treponema pallidi and varicella zoster virus antigens in the lesional skin specimens of congenital syphilis and herpes zoster. These deposits could not be clearly seen in conventional images due to out-of-focus fluorescence and background glow from peripheral tissue. More data, thus, were obtained than possible from conventional epifluorescent microscopic images. Consequently, CLSM was found useful clinically and for dermatological research.
