腹膜炎を契機として発症した空腸原発悪性garastrointestinal stromal tumor (GIST)の1例

We experienced a case of malignant gastrointestinal stromal tumor of the small intestine associated with perforated peritonitis. The patient was a 59-year-old man with had pyrexia and abdominal pain. A tumor with an edema in the left upper abdominal area was confirmed by abdominal ultrasonography and computed tomography. An emergency laparotomy was perfomed with a diagnosis of peritonitis. A tumor 7 cm in diameter was found in the jejunum and its center was perforated. microscopic diagnosis was leiomyosarcoma of the jejunum with hematoxylin and eosin stain of a resected specimen. Immunohistochemically, the tumor showed a negative response for α-smooth muscle and S-100 protein along with strongly positive responses for c-kit. Then, GIST of the small intestine was definitely diagnosed. The malignant potential of GIST is undetermined, but the treatment of choice is surgical resection.
