Diabetes changes the fluid dynamics of the blood. In particular, it increases yiscosity and makes the erythrocytes less deformable and more prone to hypercoagulation. Sialic acid has been reported to account for 95% of the negative charge at the erythrocyte surface. The present study measures sialic acid in the RBC membranes of diabetics using an enzymatic method. The results were as follows: 1) Plasma and platelet sialic acid content was high in diabetics. 2) RBC membrane sialic acid content was lower in diabetics than in normal controls. 3) RBC membrane sialic acid content did not correlated with diabetic control parameters such as FPG, HbA_1, or HbA_<1c>. 4) RBC membrane sialic acid content was particularly low among diabetics with complications or ketoacidosis. 5) The sialic acid content was lower than normal both in old RBC membranes and in young RBC membranes of diabetic patient. These results show that RBC membrane sialic acid content can serve as an index of the severity of diabetic complications. We assume that sialidase and / or sialyltransferase activity plays a role in decreasing these sialic acid content in diabetes.
