We encountered a 14-month-old girl with asplenia who died of fulminant pneumococcal infection 2 hours after consultation. Inappropriate administration of an antibiotic for a few days prior to the onset of the episode may have been etiologically involved. Previous studies have reported the relationship between asplenia and pneumococcal infection; however, prophylactic administration of antibiotics such as pneumococcal vaccine and penicillin is not always effective, and administration of an antibiotic effective for Streptococcus pneumoniae, especially penicillin-resistant Streptococcus pneumoniae, should be immediately started when infection symptoms are observed.
