大動脈の構成成分の部位別差異及び腹部大動脈瘤壁についての組織学的検討

In this study, pathological exploration was performed in various segment of the entire aorta and abdominal aortic aneurysm. Specimen were obtained from 5 segments of 10 normal whole aortae from autopsy specimens (41-72 yrs old) without Arteriosclerosis Obliterans (ASO) and Abdominal Aortic Aneurysm (AAA), and from 16 operative aneurysmal walls (55-88 yrs old) at the maxium diameter level. All specimen were fixed in formalin. In each specimem, we measured the area of the structural fibrous components (elastic fiber, smooth muscle fiber and collagen fiber). In the normal whole aortic specimen, elastic fiber area ratio (E/W ratio), collagen fiber area ratio (C/W ratio) and smooth muscle fiber area ratio (S/W ratio) to the whole specimen area were 33.9±8.8%, 26.4±26.0% and 21.2±7.4% in the ascending aorta (AscA), 31.1±12.8%, 24.0±9.7%, and 22.3±8.7% in the aortic arch (AArc), 29.8±11.4% 21.2±12.8%, and 20.7±10.3% in the descending aorta (DesA) and 29.1±10.3%, 29.1±14.9%, and 24.0±10. 5% in the proximal abdominal aorta (ProAA), and 21.9±11.2%, 30.3±15.3%, and 26.9±11.9% in the distal abdominal aorta (DisAA). E/W ratio in DisAA was lower than that in AscA, AArc, DesA. There was no significant difference between DisAA and ProAA. There was no significant difference in C/W ratio and S/W ratio among each segment. In comparison with normal (n=3) and aneurysmal aortic wall from the age matched specimen, results were : E/W ratio was 25.1±7.6% v. s. 7.1±3.8%, and C/W ratio was 25.8±13.2% v. s. 9.2±3.9%. E/W ratio and C/W ratio were higher in the normal aortic wall than in the aneurysmal wall. S/W ratio was higher in the aneurysmal wall than in the normal wall. There was negative correlation between aneurysmal diameter and E/W ratio (r=0.79, p<0.0001) but neither between aneurysmal diameter and C/W ratio nor between aneurysmal diameter and S/W ratio. As results, 1) E/W ratio in DisAA was lower than that in AscA, AArc, DisA. 2) In comparison with normal and aneurysmal aortic wall, E/W ratio was 25.1% v.s. 7.1%, and C/W ratio was 25.8% v. s. 9.2%. 3) There was negative correlation between aneurysmal diameter and E/W ratio.
