Background : In acute mesenteric ischemia, it has been difficult to assess the viability of ischemic intestines. The purpose of this study was to investigate whether it was possible to distinguish between viable and nonviable intestines by charge-coupled device (CCD) microscopy. Methods : Rats were subjected to occlusion of either the superior mesenteric artery (SMA) or the superior mesenteric vein (SMV) for 30 to 120 minutes, followed by reperfusion for 30 minutes. Microcirculation in the intestines was observed with a CCD microscope and indices for Vmax (the maximum velocity) and S (the ratio of the effective area of vascular bed to the total area of vascular bed) were evaluated. The animals which survived for 7 days were assigned 'a' and which died within 7 days as 'd'. Then each animal was assigned to one of the following 4 groups : arterial occlusion-alive (Gr. Aa), arterial occlusion-dead (Gr. Ad), venous occlusion-alive (Gr. Va), or venous occlusion-dead (Gr. Vd). Results : Both Vmax and S were significantly less in Gr. Ad than in Gr. Aa and significantly less in Gr. Vd than in Gr. Va. While there were no significant differences in Vmax and S between Gr. Aa and Gr. Va, those were significantly less in Gr. Vd than in Gr. Ad. In arterial occlusion group, no rats died when Vmax exceeded 1150 and S, 0.92 but none survived when Vmax was below 990 and S, below 0.84. In venous occlusion group, no animals died when Vmax exceeded 730 and S ,0.76 and none survived when Vmax was below 610 and S, below 0. 66. Conclusions : Vmax and S with a CCD microscope were useful in estimating viability of intestines in acute mesenteric ischemia.
