Recently in Japan, many young Medical Socialworkers have lost professional confidence because of fundamental changes in medical welfare, so called 'The Big Bang in social welfare' in the late 1990s the structural reform of the social security system started to improve overall efficiency through reorganization across systems. Public care law for the elderly is a typical result of these reforms. In clinical settings, care managers took on new professional roles in the care of the elderly. Their main task is to arrange services to satisfy the needs of users and to give consultation to users and their families. These tasks have also been carried out by medical social workers as a core responsibility for a long time. This historical reality has caused conflict and an inadequacy of professional identity for medical social workers especially those with limited experience. In this article, the currend roles of medical social worker are reexamined by comparing them with their role as indicated in 1980 by NASW. This article concludes that workers have to recognize that professional identities and the roles of the medical social worker must be responsive to the situation of their society because jobs of suchs workers currently have a strong interconnection with medical and scientific technology.
