The Current State of Chronic Dialysis Treatment in Denmark and Japan

Background : The dialysis patients in Japan is accounting for nearly 25% of the dialysis patients in the world. Denmark and Japan have the similar mentality toward the elderly. So, this report describes the current state of chronic dialysis treatment in Denmark and Japan. Methods : The Danish Society of Nephrology and the Japanese Society for Dialysis Therapy conduct a questionnaire survey of virtually every dialysis facility each year. The source of materials studied were reports published by the society. Results : In Denmark the prevalence of patients were treated by maintenance renal replacement therapy and of with a functioning renal graft were 409 and 265 per million inhabitants, respectively in year 2000. On the other hand, in Japan the prevalence of patients were treated by maintenance renal replacement therapy was 1,624 per million inhabitants in 2000. In Denmark the distribution of new patients on treatment of end-stage renal disease (ESRD) in 2000 were nearly 60% on dialysis and 40% on renal transplantation (nearly 80% cadaver and 20% living donors). In Japan new ESRD patients almost were treated by dialysis. In both country the proportion of chronic glomerulonephritis (CGN) decreased and the proportion of diabetic nephropathy (DM) increased from year to year. The proportion of patients initiating of dialysis of age over 60 was nearly 54% in Denmark and 66% in Japan. Direct causes of death in dialysis patients were mainly cardiac, infection, vascular and malignancy both in Denmark and Japan. Conclusion : The prevalence of the ESRD patients per million inhabitants in Japan was nearly four times as many as in Denmark. Nearly 99% of these patients were treated by dialysis in Japan. On the other hand, at the end of the year 2000 nearly 60% of ERDS patients (3,655 patients) were on dialysis and 40% had a functioning renal allograft in Denmark.
