At nursing homes for senior citizens (nursing facilities) suffering from physical handicaps and dementia, it is said that most residents are in need of help in oral self-care. Giving due attention to oral care seems to be an important task on the part of care providers. Our nursing facility has taken a positive approach to this task as well, ever since its opening in August, 2002, by installing an in-house dental clinic where a dental hygienist is present. Based on observation over the course of two years of the nursing facility's operation, the performance of our service is reported herewith. The subjects were 167 residents (average age of 82.7±8.3) with residency of over 12 months (51 males 78.8±9.5 and 116 females 84.5±7.2). Each of the subjects was given an oral cleanliness check by a dental hygienist at his or her admission to the facility and tailored care plans have been prescribed and implemented. For the evaluation of oral cleanliness, the Simplified Oral Hygiene Index (OHI-S) has been employed. The results are as follows : 1) The average oral cleanliness index at admission was 3.6, which improved to 1.9 after 12 months. 2) As to diet preparation conditions, those taking standard diet at admission were 97 (58.1%) and those taking minced diet were 59 (35.3%); the figures significantly improved to 120 (71.9%) and 42 (25.1%) respectively after 12 months. 3) Of the subjects, 118 (70.7%) experienced dental treatment. 4) In order to provide high quality oral care, the staff must be well aware of the needs of oral care. 5) A dental hygienist is in a position to give proper oral care guidance to nurses and care workers in addition to providing effective oral care.
