Treatment of a patient with severely crowded Class I malocclusion by extraction of the mandibular lateral incisors

The treatment of Class I malocclusion with severe mandibular crowding is presented. The mandibular left lateral incisor was obstructed, and crowding existed in the anterior region of the mandibular arch. Both the mandibular lateral incisors and the maxillary first premolars were extracted. An 0.022 inch preadjusted edgewise appliance was used. For aesthetic reasons, the placement of the brackets for the mandibular canines were intensively crown-mesial angulated. Thus, the aesthetic problem of incisor alignment was improved. An ideal overjet, overbite, and Class I molar relationship were established. After that, retainers were placed. Although root proximity remained between the central incisor roots and canine roots, three-year retention shows that a good alveolar bone level was maintained.
