弁膜症手術における blood cardioplegia と crystalloid cardioplegia の比較検討

The protective effect of blood and crystalloid cardioplegia on the ischemic myocardium was studied in clinical cases undergoing valvular surgery. Multidose potassium (K=30mEq/L) blood cardioplegia was employed in 15 patients (BCP group), and multidose potassium crystalloid cardiopegia (St. Thomas' Hospital solution) was employed in 79 patients (CCP group). Re-do operation was more frequent in BCP group than in CCP group (p<0.01). Myocardial temperature was significantly higher in BCP group than CCP group (BCP11.1±1.0℃versusCCP 8.7±0.1℃ ; p<0.01), but rectal temperature was significantly lower in the former (BCP25.7±OSC versus CCP 27.2±0.1℃; p<0.01). Ishemic injury, as estimated by the highest value of creatine kinase MB (CK-ME) or entire time coure of CK-MB was significantly (p<0.05) less in BCP group than CCP group (highest CK-MB; BCP 67.9±14.9IU/L versus CCP 96.2±6.4IU/L). However, there was no definite relationship between aortic cross clamp time and post operative CK-MB level in both group. All of the nine patients (2 patients in BCP group and 7 patients in CCP group) who showed an elevated CK-MB level exceeding 150IU/L suffered from hemodynamic disorder in post operative course. The incidence of low out syndrome and operative death was not different between two groups. We conclude that blood cardioplegia is superior to the crystalloid cardioplegia in myocardial protection during cardiac arrest at moderate hypothermia in valvular surgery.
