Intra capsular pathoses as a result of temporomandibular joint (TMJ) disc displacement were reported as follows : synovitis, deformation of the disc, fibrous adhesion and narrowing of the joint compartment, with or without bony structual change. Among these conditions, fibrous adhesion is most likely to occur in the lateral area of the cavity and may require arthroscopic lysis of the adhesion. However, it was reported arthroscopic surgical intervention can also result in narrowing of the joint compartment because of postoperative fibrous adhesion as can an open arthroplasty. Therefore, prevention of intra capsular adhesion following the surgery is essential. This study evaluated the effect of inseerting fibrin glue in the TMJ compartment to prevent the fibrous adhesion. Though a fibrin glue is a biomaterial wideused as an adhesive tissue, (1) it has no adhesiveness in a moistend room, (2) it can be injected in the joint cavity and readily coagulates, (3) the coagulate is absorbable, resulting in temporary use. The experimental animals used in this study were 84 adult male rabbits weighing about 3-4kg these were divided into three groups ; A. experimental group, 40 animals : the upper compartment of the TMJ was surgically opened and 0.2ml of fibrin glue was injected. Then the joint was closed. B. sham experimental group, 40 : the upper compartment of the TMJ was surgically opened and readily closed. C. untreated group (control), 4 Animals in A. and B. were sacrificed on the first, third, seventh, 14th, 21st, 28th, 42nd, 56th 84th, and 112nd day after the surgery. Their TMJs were extirpated with the surrounding tissues, and histopathological observation was made with nondecalcified and decalcified specimens stained with Hematoxylin-Eosin and phosphotungstic acid and hematoxylin. The results were as follows : 1) Fibrin glue inserted in the cavity remained only at the lateral and posterior recess from the initial stage of observation. 2) On the first and third day, the mass of fibrin glue was circumscribed with a granulative tissue including fibroblasts and synovial cells. 3) From the seventh to the 14th day, the fibrin mass was surrounded by a fibrous connective tissue. 4) From the 28th day to 56th, most of the fibrin mass had disappeared and been replaced by granulation tussue. 5) From the 84th day to 112nd, the microscopic views in the group A. and B. were similar to the findings observed in the untreated group. 6) There was no fibrous adhesion in the upper joint cavity nor deformation of body and cartilageous structure in any animal through all stages. 7) Accordingly, fibrin glue inserted in the upper joint cavity remained in the lateral and posterior recess of the cavity for at least seven days without any foreign body reactions. These findings indicate that fibrin glue can be used as a separator to prevent fibrous adhesion in a joint cavity.
