The application of distraction osteogenesis of the upper and lower arches at midlines to retreat bicuspids extraction case with successful outcome in occlusion and profile was presented in this paper. The diagnosis was classified as Skeletal Class II jaw base relationship with constricted upper and lower arches and protrusion of maxillary anteriors. Upper and lower midline osteotomies were carried out and both arches were expanded by using expansion screw. At the expansion rate of 0.45mm per day, 6.0mm for the upper and 4.0mm for the lower were gained by outpatient visit style. In a retreatment with earlier first bicuspids extraction, the proposed bimaxillary midline split with subsequent distraction osteogenesis permit bodily bone expansion and stabilization with relatively short healing period.
