口腔外科手術侵襲度の新しい指標に関する研究 : 特に血中サイトカイン濃度の変動を中心として

Recently, the role of cytokines in surgical stress and subsequent acute phase has gradually been clarified^<1,3-18,28-37>. This study (1) investigated whether interleukin-6 (IL-6) and granulocyte colony-stimulating factor (G-CSF) in blood can be used as an index of surgical stress (2) and determined which factors influenced the levels of these cytokines by comparison with other indices. The subjects consisted of forty patients (ASA I and II, aged 17-69 yrs) undergoing oral surgery under general anesthesia at Fukuoka Dental College Hospital. Cases were allocated to the following three groups, according to the possible intensity of surgical stress, i.e. group A (simultaneous bimaxillary orthognathic surgery) group B (sagittal splitting osteotomy of ramus) and group C (maxillary sinusectomy). Changes in IL-6,G-CSF, and other indices were measured and compared in each group. Results were as follows; 1. Blood IL-6 and G-CSF concentrations increased immeditately after surgery, depending on the magnitude of surgical stress. 2. Changes in neutrophil counts showed a similar tendency to changes in those of IL-6 and G-CSF concentrations. 3. The lymphocyte count decreased in both group A and group B, immediately after surgery. 4. Plasma epinephrine concentration increased at 30 and 120 min after the start of surgery and immediately after surgery, depending on the magnitude of stress. 5. Blood IL-6 and G-CSF concentration did not seem to be influenced by differences in anesthetic agent (between N_2O-O_2-sevoflurane and NLA), volume of blood loss and duration of surgery. 6. Plasma epinephrine concentration was maintained at a higher level in high volume blood loss group than that in low volume blood loss group. From the above findings, increases in blood IL-6 and G-CSF concentrations seemed to be related to a scale of "tissue damage", a factor of surgical stress (stressor), and be can use as indices of surgical stress in oral surgery. Moreover, the neutrophil count may also reflect the magnitude of surgical stress.
