This study was designed to investigate the wound healing process of the residual pulp, bone defect and apical seal after mesial apicoectomy in the dog lower mandibular third and fourth premolar. Twenty six dogs were used, and the hydroxyapatite (HAP) granules were filled in the wound cavities of eight dogs. In this model, periapical tissues and pulp were histopathologically and radiographically observed between 7 to 120 days. The results were as follows : 1. In the apicoectomized group, the wound cavity was repaired with newly formed alveolar bone after about 30 days. The orifice of root canals was spontaneously closed with hard tissues after about 90 days. 2. The canal orifice was closed in either of the two type : Type I by the addition of bone-like cementum to the root canals and type II by hypertrophy of the central root canal wall. 3. Type I closure was predominant in young teeth with a large pulp and a large root canal foramen. Both type I and type II had a similar incidence in mature teeth with narrowed pulp and root canal foramen. 4. In the HAP-filled group, the canal orifice was spontaneously closed with hard tissue after 90 days, similar to the apicoectomized group. Although both types of closure were also seen in this group, postoperative formation of the periodontal membrane was not seen and wound healing tended to be delayed in this group. 5. The incidence of type I closure of young teeth did not significantly differ between the HAP-filled group and the apicoectomized group. However, the incidence of type II closure of mature teeth was markedly higher in the HAP-filled group, as compared to the apicoectomized group. 6. It is seemed that type I closure occurs in cases where blood is adequately supplied from the pulp of the unoperated healthy root to the pulp of the root canal and from the wound cavity to the pulp, and that type I closure occurs in cases where blood supply to these regions is inadequate. 7. A single nerve fiber began to grow from the vicinity of the wound cavity 7 days after apicoectomy. Nerve fibers had grown into the wound cavity by the 21st day after apicoectomy. 8. As compared to the HAP-non filled group, the HAP-filled group had less nerve fasciae between HAP granules and between granulation tissues of the bone trabeculae, resulting in delayed healing.
