It occured to us that, to know the dynamics of blood circulation in the heart must be a prerequisite for taking chest plain radiograms of the patient under CCU at the optimum radiographing conditions. For verification we compared the chest plain radiograms and the results of examinations of the right heart catheterization by means of the Swan-Ganz catheter made on some 140 patients of acute myocardial infarction and of heart failure under CCU who were received by our hospital and got the following results : (1) In the case of the acute myocardial infarction patients, approximately 80% had P.W.P.s (pulmonary arterial wedge pressure) of 18 mmHg or lower. As to the case of heart failures, approximately 64.9% had P.W.P.s of 18 mmHg or higher. (2) For 19.5% of the patients of acute myocardial infarction and 68.2% of those of heart failure, pulmonary congestion was noticed. (3) A total of 71.7% of the patients of acute myocardial infarction had C.V.P.s (central venous pressure) of 10 mmHg or lower, and P.W.P.s 18 mmHg or lower. (4) For 4.3% of the patients of acute myocardial infarction, accumulation of pleural effusion was noticed. (5) In the case of heart failures, 8 among the 65 patients had vanishing tumors, which were noticed on the right thorax with effusion and cardiac index 2.2 l/min/m^2 or less in all the 8 cases. In the chest plain radiography under CCU it is important to take the above-listed phenomena into consideration. Radiographing conditions under which the radiograms were taken are useful for next opportunities and should be communicated to the succeeding radiographers.
