ペンシル型CCD生体顕微鏡システムを用いたin vivo肺微小循環と個々の肺胞の同時可視化と機能解析

【目的】ペンシル型CCD生体顕微鏡を用い、in vivoにてラットでの肺胸膜直下微小循環を直接観察し、呼吸条件の肺循環へ及ぼす影響を解析する。【方法】肺微小循環と個々の肺胞の動的イメージを呼気終末陽圧(PEEP 20 cm H_2O)の有無や一回換気量の異なる条件下(2ml〜5ml)で記録し、毛細血管の径や赤血球の速度、肺胞の径について解析を行った。【結果】1回換気量が2mlのとき、肺胞径52±22μm、毛細血管内の赤血球流速は760±200μm/secであったが、1回換気量を5mlとすると肺胞径は78±22μmとなり、赤血球速度も50±6.0μm/secへと著明に減少した。1回換気量5ml、PEEP 20cmH2Oの呼吸条件下では、毛細血管内だけでなく細動静脈の血流も吸気終末には一時停止した。【結論】ペンシル型生体顕微鏡により、人工呼吸管理下におけるラットの肺微小循環及び肺胞の動的イメージングをin vivoで可能にし、呼吸状態と肺微小循環の関係を生理的条件下で明らかにした。
