The Adacolumn is an extracorporeal granulocytes and monocytes adsorption column. The Adacolumn contains bead-type adsorbent made from cellulose diacetate. The beads are 2 mm in diameter and 60 degrees in contact angle with water. This column removes 20-40% of granulocytes and monocytes from introduced peripheral blood. The removal rate for lymphocytes and platelets is about 5%. Usually blood is drawn from a vein and is returned via another vein. Blood flow rate is 30 mL/min and the extracorporeal method consists in direct hemoperfusion. The Adacolumn has been applied to various kinds of autoimmune diseases and inflammatory diseases, and is now confirmed effective for rheumatoid arthritis and ulcerative colitis. In Japan, the population of patients with ulcerative colitis has been increasing and is mainly treated with steroid. However, there are many steroid-resistant patients and the side-effects of steroid cause serious problems. Therfore, the Adacolumn will be the next standard treatment for ulcerative colitis.
