The estimation of metachromatic reactions (MR) in the urine was made in 27 male and female cases of renal tuberculosis and cystophthisis, 77 cases of urosis, and 86 cases of pulmonary tuberculosis. Cases of renal tuberculosis and systophthisis were classified into two groups : one group in which chemotherapy was performed in conjunction with an operation (group 1), and the other group which received chemical treatment alone (group 2). Long-term observations on the healing processes in both groups led to the following conclusions : 1) The pre-treatment MR of renal tuberculosis and cystophthisis showed a maximum of an eightfold value in both sexes. 2) For group 1, postoperative MR values became negative one month later at the earliest and eight months afterwards at the latest. 3) The negativity of MR in group 2 occurred 14 months later at the earliest and 36 months afterwards at the latest. When MR became negative in each case, the picture of the pelvis renalis was improved on at the same time. 4) When the negativity of MR was noted in the urine, improvements were seen, including decreases in red and white blood cell counts, as compared with findings at the onset of treatment. There was a decrease in the MR values after tubercle bacilli disappeared from the urine even on cultivation. 5) The slower occurrence of MR negativity in group 2 than in group I seems to be due to the long-term continuous excretion of an MR-positive substance, an ester type of higher fatty acid, into the urine. 6) For the assessment of the effects of treatment in renal tuberculosis effects of treatment in renal tuberculosis and cystophthisis, the MR qualitative method alone is not sufficient, but the estimating technique should be adopted for observation of healing processes.
