顎口腔領域における扁平上皮癌患者の血清 Immunosuppressive Acidic Protein (IAP) に関する臨床的研究

There have been a large number of reports on the clinical usefulness of tumor markers for various malignant tumors in many medical fields, and these tumor markers are known as tumor-mediated substances, but not specific substances directly produced by a tumor tissue itself. Immunosuppressive acidic protein (IAP), which was first detected by Matsuda et al. in 1977 from the serum of the tumor-bearing mouse, has been widely used as an immunological parameter to observe clinical courses of cancers occurring in the digestive system, lung, breast and kidney. Seventy patients with squamous cell carcinoma in the oral cavity were therefore examined for serum IAP level and clinical evaluation was made. The results were as follows : 1. The average serum IAP of 70 patients with oral cancer (squamous cell carcinoma) was 594.9±266.8μg/ml and the positive rate was 67.1% (47/70 cases). In contrast, the average for control patients with benign oral disease was 349.4±113.9μg/ml and the positive rate was 12.9% (4/31 cases). Statistically significant difference was found. 2. In serum IAP levels, tongue cancer and gingival cancer of upper jaw were at higher levels than another oral cancer. 3. With progress of clinical stage, namely, as the tumor grew and the lesion developed, serum IAP level gradually increased its average and positive rate. Also, in comparison with primary cancer, advanced cancer showed higher values in many cases. 4. By prognosis, the average of serum IAP level and positive rate for good prognosis cases were 523.6±216.3μg/ml and 55.2% (16/29 cases), respectively. For poor prognosiscases, they were 642.6±292.0μg/ml and 73.2% (30/41 cases), and statistically significant difference was found. 5. As to change in serum IAP level before and after treatment, serum IAP level tended to decrease and become negative after treatment for the cases of good prognosis. For the cases of poor prognosis, serum IAP level was unchaged or increased in many and tended to become negative. With imminent death especially, serum IAP level was markedly high. 6. In these investigations, serum IAP level well reflects the extent of the tumor and severity of condition. Serum IAP level is thought to be a useful indicator for estimating progress, effect of treatment, and prognosis. Its clinical utility is considerable.
