歯科治療時における患者の不安と生理的変化の関係についての研究 : 不安テストと循環動態および血漿カテコールアミンからの評価

This study investigated relationship between anxiety and psychophysiological response during dental treatment under local anesthesia. Twenty eight subjects in good general health and between 20 and 26 years old (mean age 21.8) were selected. They had undergone State-Trait Anxiety Inventory (STAI) that evaluates both of state anxiety and trait anxiety. Plasma catecholamine concentrations were measured, and blood pressure, pulse rate, stroke volume and total peripheral resistance were recorded. The procedure of local anesthesia was undertaken in double-blind crossover design with 2% lidocaine containing 1 : 80000 adrenaline or 2% lidocaine only. An observation was made on the changes of cardiohemodynamics and plasma catecholamine. Low anxiety group who had gotten lower scores on STAI showed no significant differences in hemodynamic changes after anesthesia in the use of neither lidocaine containing adrenaline nor lidocaine only. High anxiety group who had gotten higher scores on STAI revealed significant differences in hemodynamic changes after injection. Systolic blood pressure, pulse rate and stroke volume increased significantly from the baseline, and total peripheral resistance decreased significantly. When applying anesthesia with lidocaine containing 1 : 80000 adrenaline, blood concentrations of adrenaline (AD), noradrenaline (N-AD) and dopamine (DOP) increased significantly after injection in the high anxiety group. Whereas, in the low anxiety group, only the concentration of AD increased significantly after injection. When applying anesthesia with lidocaine only, blood concentration of AD, N-AD and DOP increased significantly after injection in the high anxiety group. Whereas, in the low anxiety group, plasma catecholamine levels remained unchanged. In the high anxiety group, blood concentrations of AD and N-AD showed higher level than low anxiety group at the baseline. This study suggested that emotional stress as fear or anxiety has influenced in the hemodynamic and plasma catecholamine responses during dental treatment. And it is considered that STAI has some potential in clinical dentistry.
