結晶化ガラスの歯周治療への応用 : 犬の実験的骨欠損に填入後の組織学的検索

The purpose of this study was to evaluate the biocompatibility and the osteoconductivity of CaO-P_2O_5-MgO-SiO_2-CaF system glass ceramics and to determine its usefulness as bone graft material in periodontal therapy. Seven adult beagle dogs, aged 12 to 15 months, were used for this study. Prior to implantation of the glass ceramics, the second and fourth premolars of the mandible were extracted. Three month after extraction, artificial three-wall bony defects were made at the mesial side of the third premolars and the first molars. The glass ceramics were implanted into the bony defects adjacent to the first molars. The dogs were sacrificed at 2 weeks, 1, 2, 3 and 6 months, and 1 year after implantation. The implanted sites were compared with the control sites which were not filled with the glass ceramics histopathologically and radiologically. The results were as follows : 1. The wound healing of soft tissue was completed after 1 month in both the control and experimental sites. The connective tissue around the glass ceramics showed no signs of inflammation. 2. Newly formed bone tissues and osteoids, which had grown from original bone constitution, showed tight bonding with the glass ceramics at 2 weeks after implantation. 3. One month after implantation, macrophage-like cells in which phagocytosis of the calcified particles was observed in the cytoplasm appeared around the glass ceramics. The number of them increased with time until 3 months and then decreased. 4. Since 2 months after surgery, the bone regeneration in the implanted sites was slower in progress than that in the control sites. However, at 1 year after implantation. Haversian systems were formed in the regenerated bone including glass ceramics, also the contour of regenerated bones showed shallower hollow than that of the control. These results indicate that glass ceramics have good biocompatibility and high osteoconductivity. Considering these properties there is possibility that the glass ceramics provide the scaffold for bone regeneration. Based on the above study it is suggested that the introduced glass ceramics is useful and practical as bone graft material for the restoration of alveolar bone as physiological structure in periodontal therapy.
