After single crystal aluminum oxide implant was inserted on the mandibles of adult monkeys, the implant head was cemented to the superprostheses. Under occlusal function, the effect of oral hygiene was examined clinically, roentogenographically, histopathologically and histometrically, from one month to six months after implantation. The results were as follows. 1. On plaque index and gingival index, the scores of non-cleaning group were higher than those of cleaning group. The measurements of gingival crevicular fluid of non-cleaning group were also higher than those of cleaning group. 2. Roentogenographically, in both 1-month and 3-month cases after implantation there was little difference between non-cleaning group and cleaning group. In 6-month cases after implantation there was slightly greater osseous sauserization in non-cleaning group. 3. In both non-cleaning group and cleaning group, the attached epithelium adjacent to the implant, composing only 2-3 layers of squamous cells, were flatly ranged along the implant surface and no prominent hyperplasia or proliferation was observed. 4. In non-cleaning group in 6-month cases infiltrative connective tissue adjacent to the attached epitheum extended to the deep portion of the lamina propria. The junction among the attached epithelium along the implant was not tight and inflammatory cells invaded the epithelium. 5. On non-cleaning group in both 3-month and 6-month cases, bone resorption was observed at the top of the alveolar crest. 6. At the screw thread area there was little difference between non-cleaning group and cleaning group. In both non-cleaning group and cleaning group at 6-months the implant was in direct contact with the mature compact bone. 7. On the percentage of infiltrative connective tissue in the lamina propria of the gingiva surrounding the implant, non-cleaning group and cleaning group showed significant differences in all of 1-month, 3-month and 6-month cases. Both non-cleaning group and cleaning group showed statistically singnificant increases in 6-month cases than in 1-month cases. 8. On the percentage of downgrowth of the attached epithelium adjacent to the implant, the degree of non-cleaning group showed higher percentages than those of cleaning group. Both of non-cleaning group and cleaning group showed statistically significant increases in 3-month cases and 6-month cases than in 1-month cases. On the basis of the above findings, it was proved that oral hygiene greatly affected the wound healing of the surrounding tissue of the implant, and the importance of oral hygiene after implantation was re-recoganized.
