強制的下顎位の変化に伴う咬合接触時間および下顎の復位力の測定に関する研究

An activator is an appliance to activate functional force of the muscle and this force is used as an orthopedic force. In order to activate this force, it is necessary to change mandibular position with use of an activator and use the induced force returning the mandible to the original position as an orthopedic force. However, a suitable amount of change in the mandibular position to activate the force has not yet been clearly described to date. Therefore, as a means of studying this relation, a measuring appliance was made by modifying the activator. By using this appliance, measurements were taken of occlusal contact duration while the subjects bit the appliance in the vertical direction and of mandibular restituting force to the original position in the anteroposterior direction. In measuring the occlusal contact duration, six mandibular positions were selected : displacing the mandible 4mm and 8mm vertically and 0mm, 4mm and 8mm forward for each vertical displacement. In measuring the mandibular restituting force to the original position, four mandibular positions were selected : displacing the mandible 4mm and 8mm vertically and 4mm and 8mm forward. A small switching device was fixed at the site corresponding to the premolar on the occlusal surfaces of the activator for measuring the occlusal contact duration and a strain gauge transducer was attached to the guide wire of maxilla for measuring the mandibular restituting force to the original position. The subjects for this study were six Japanese adult males with normal occlusion. Four hours were allocated for measuring to each subject with first one-hour in sitting posture in a waking state, second one-hour in lying posture in a waking state, third one-hour in lying posture in a sleeping state and the final one-hour in sitting posture in a waking state, in sequence. First 30 minutes of each hour was not used for the measuring purpose. Second 30 minutes was used for this study and converted into one-hour period. The results were as follows : 1. With the same amount of forward displacement of the mandible, greater the vertical displacement, longer the occlusal contact duration was. However, forward displacement of the mandible was not closely related with the occlusal contact duration. 2. The results of the mandibular restituting force to the original position were as follows : 1) The mandibular restituting force to the original position with use of an activator was greater in the sleeping state than in the waking state. 2) Greater the vertical or anteroposterior displacement, greater the mandibular restituting force was. 3) In comparison of the mandibular restituting forces with use of an activator in sitting posture before sleeping and after sleeping, the mandibular position with a smaller degree of mandibular displacement showed smaller mandibular restituting force after sleeping in sitting posture.
