The present paper is concerning on the thiamine metabolism before and after the portacaval anastomosis in the patients with portal hypertension. After the subcutaneous injection of 20mg of thiamine to the patients with portal hypertension, who were belonged to the 2nd and the 3rd group by prof. Imanaga's classification, fluctuations of total and bound thiamine in the blood and of thiamine in the 3 hours' urine were examined. At the same time, pyruvic acid in the blood was also determined. It was revealed that the thiamine metabolism in the 2nd group were similar to that in normal group, meanwhile, in the 3rd group, free thiamine in the blood and in the 3 hours' urine were increased after the surgery. When symptoms of portal systemic encephalopathy, "Eck fistula syndrome" developed, free thiamine in the blood and in the 3 hours' urine were remarkably increased. An administration of thiamine diphosphate or thiamine derivatives may be recommened to restore the abnormal thiamine metabolism in the patients of 3rd group.
