We developed a new applicator for endobronchial irradiation using ^<192>iridium thin wires and tested its clinical application in 21 cases of chest X-P negative lung cancer. The applicator could be advanced to the target bronchi in all cases, and the treatment had to be discontinued only 2 times. Recurrence was observed in 2 of the 21 cases. No complications of this tretment were observed. Endobronchial irradiation has been applicable only to larger bronchi by conventional techniques and has been used primarily as a palliative treatment. However, this applicator can be readily and selectively advanced up to subsegmental bronchi, and the spacers placed at its tip, where the radiation source is contained, improve the dose distribution. This technique is considered to be particularly useful for the treatment of early stage central type lung cancers.
