Pulmonary arterial aneurysms are very rare. This is a case report of a pulmonary artery aneurysm with a fistula to the right bronchus developing massive hemoptysis. Bronchoscopy was performed on the operating table but the bleeding had already stopped and the bleeding point could not be found. CT scan of chests and cardiac catheterization were also performed. There was no pulmonary hypertension. Elective right thoracotomy revealed aneurysm of the pulmonary artery in the right lower lobe. Right bilobectomy (lower and middle lobes) yielded good results. The pathological findings revealed that the wall of the pulmonary artery was very thin. The wall of the pulmonary artery could not be found in one part of the pulmonary artery at the site of the fistula. There was a thrombus inside the pulmonary artery.
