In order to investigate exertional electrocardiographic-hemodynamic correlation serial changes in the exercise factor were observed by means of the direct Fick method on supine bicycle exercise in 36 cases with various cardiac conditions. The following conclusions were drawn from these observations. 1. The pattern of serial changes in the exercise factor was characteristic of ischemic and/or hypertensive heart disease. 2. The exercise factor revealed a good correlation with ST-T responses to exercise, so that an optimal work load or commenssurate cardiac function could be reasonably estimated according to the method proposed by N. KIMURA. 3. There was one exceptional case with angina of effort in which a discrepancy was observed between the exercise factor and ST-T responses. In this case, however, serial changes in EF were not proportional to simultaneous changes in ratio of myocardial oxygen consumption increasing ratio to that of total body postulated by modified tension time index, suggestive of the presence of well functioning collaterals in coronary circulation. 4. A work intensity of 1.0kilo-pond 30km/hr was equivalent to 4.7±0.23 as the relative metabolic rate in this study.
