Hemodynamic responses to exercise were studied in 67 patients with coronary heart disease. A consistently diminished response to exercise was demonstrated in patients with antero-lateral infarction. Patients with posterior and anterior infarction as well as those of classic angina with severe and frequent attacks reacted abnormally when either precordial pain or marked S-T displacement actually developed. No difference was appreciated in hemodynamic response to exercise between patients with mild or occasional attacks of angina and the control. Patients of atrial fibrillation with enlarged heart also responded abnormally to exercise. It was noted that hemodynamic consequences during exercise of coronary heart disease depended upon the presence and extent of previous infarction, the severity and frequency of anginal attacks, development of actual chest pain or S-T displacement on exertion, attendance of atrial fibrillation and heart size. Finally, the relation of cardiac size during exercise to hemodynamic abnormalities was evaluated and the significance of abnormal distension of systolic heart size in hemodynamic abnormalities was discussed.
