Probable Lymphocytic Hypophysitis Diagnosed by Short-term Serial Computed Tomography and Gallium-67 Scintigraphy : Case Report

症例は61歳女性、頭痛、全身倦怠感、左動眼神経麻痺にて発症。CT所見では、鞍内外へ進展した腫瘤を認めた。内分泌学的検査上、下垂体機能低下を認め、ガリウムシンチグラフィーでは同部の異常集積を認めた。症状は治療なく徐々に改善し、2週間毎に施行したCTで腫瘤は徐々に縮小した。発症12週間後には腫瘤は正常下垂体大に縮小し、下垂体機能も正常範囲内に改善した。経過中のCT、MRIでは下垂体卒中を示す所見は認めず、1年後のCTでは下垂体は更に縮小しempty Sellmの所見を呈した。以上の所見より本例はリンパ球性下垂体炎と診断された。現在、患者は発症88年を経過したが、ホルモン補充療法を必要とせず、再発も認めていない。
