テントヘルニアを来した患者の予後は極めて悪いため、Kernohans Notchの画像所見は稀である。我々は外傷性硬膜下出血に起因したテントヘルニアのMRI所見について報告する。患者は16歳、女。交通事故で受傷し、右前頭側頭部硬膜下血腫に対する開頭血腫除去および外減圧術を受けた。術後、右方麻痺と右側同名性半盲が認められ、MRIで左大脳脚および左後大脳動脈領域にT1で低信号、T2で高信号の領域が確認された。症状とMRI所見より、大脳脚病変はKernohans Notchと診断された。
