Postpartum Dissecting Aneurysm of the Superior Cerebellar Artery : Case Report

• 妊娠中毒症の37歳，女性．帝王切開分娩の翌日，頭痛発作が出現した．CTスキャンでクモ膜下出血（SAH）を認め，椎骨動脈写で右上小脳動脈（SCA）起始部の紡錘状拡大とその末梢の狭窄所見を認めた．出血発作を繰り返し，初回発作の24時間以内に開頭手術を行った．右SCAの血管壁に外膜下出血を示す変色を認め，近位部に破裂部位と考えられる瘤状膨隆が存在した．術中所見よりSCAの解離性脳動脈瘤（DA）と診断した．瘤状膨隆部をクリッピングし，この部と変色したSCA近位部のラッピングを追加した．術後経過は順調で，脳血管写で瘤状膨隆の完全消失と右SCAの血流温存を確認した．SAHを繰り返す破裂DAに対しては外科的治療を積極的に行うべきである．
