Akinetic Mutism and Involuntary Movements Following Radical Resection of Hypothalamic Glioma : Case Report

症倒は3歳男児、視力障害と間脳症候群で発症. MRIでは鞍上部の巨大な腫瘍と閉塞性水頭症を認めた. 脳室腹腔短絡手術を施行後、2回に分けて腫瘍摘出術を行った. 腫瘍はpilocytic astrocytomaであった. 手術後3週目より、血清電解質異常と意識障害を発症した. 電解質を補正し、神経症状も一旦改善したか、その後、再び遵延性意識障害と、四肢硬直とジストニア様の不随意運動が頻発するに至った. 同時期のCTでは腫瘍の残存は認めず、両側の基底核に限局性低吸収域を認めた. ドーパミン作働薬を投与すると神経症状の劇的改善を見た. 本症例は視床下部原発性gliomaの術後にドーパミン代謝異常が発来し特異的な神経症状を呈したと考えられた.
