Cervical Ventral Epithelial Cyst Treated by Anterior Corpectomy : Case Report

症例は15歳男性. 前屈にて増強する頚部痛にて発症. 他覚的には左の軽度手指運動障害と深部腱反射亢進を認めた. MRIにてC4-5腹側にcystが存在し、cordは背面に薄く圧排されていた. 前方よりC4,5の維体を削除し, cyst wallを広範に切除した. 維体は〓骨にて固定した. 病理所見は壁は一肩の円柱上皮で、mucln分泌がみられ, Epithellal Cyst(EC)と診断した. ECは脊髄腹側に位置しても, 後方到達法にてcystの開放が行なわれることが多いが, 前方より維体を削除することでcystに直進可能となり, cordに一切操作を加えずに確実で広範なcystの摘出が可能であった.
