3才のDown症候群に合併した環軸椎亜脱臼による脊髄障害に対し, 簡便なone piece cervical device(OPCD)を用いて後方固定を行い, 良好な成績を得た. 幼児の頸椎は小さく脆弱であり従来のwiringは手技の面, 骨の強度の問題からこの症例には不適切と考えられたため, 金属によるinstrumentationと自家骨を用いた後方固定を選択した. また, 精神発達遅滞を伴った3才の幼児の術後の外固定が十分に行えるかどうかが大きな問題点であったが, これに対してはあご付き硬性コルセットを小児用に作成し十分な頸部の安静が保て有用であった. 術後4ヵ月で患児は介助歩行が可能となり, MRI上も上部頸髄の圧迫が消失した.
