Acute Thrombolytic Therapy and Subsequent Angioplasty for Atherosclerotic Stenosis of the Basilar Artery : Case Report

脳底動脈閉塞による急性脳幹部梗塞に対して、血栓溶解療法と経皮的血管形成術(PTA)を行い、良好な結果が得られたので報告する。症例は50歳、男性。突然の意識消失で発症し、救急搬入時は除脳硬直を示した。入院1ヵ月前より右半身のしびれ感と眩暈からなるTIAを何度か繰り返していた。脳血管造影では、脳底動脈の起始部に高度狭窄が見られ、狭窄部以降の描出は極めて不良であった。96万単位のウロキナーゼ動注により、脳底動脈以降の描出が良好となり、意識も回復した。橋の梗塞のため右片麻痺を残したが、独歩可能となったため、発症3ヵ月後に脳底動脈起始部の高度狭窄に対してPTAを行った。PTA6ヵ月後の血管造影では再狭窄はなく、社会復帰している。
