CDP-Choline, Dexamethasone投与による妊婦羊水中のSurfactant Lecithin, Total Lecithin, L/S比,およびSurfactant Titerの変化

正常妊婦の羊水199例についてtotal lecithin, sphingomyelin, L/S比,surfactant titerを測定しその変動をみた.次いで妊娠35週の妊婦にCDP-choline, dexamethasoneを投与して上記4種とsurfactant lecithinであるminimum disaturated lecithin (MDSL)を指標としてその作用を検討した. 1. 正常妊娠では,total lecithin, L/S比,surfactant titerは在胎週数がすすむにつれて増加し,妊娠38週頃で急激な増加がみられた. 2. surfactant titerとtotal lecithin, L/S比は非常に良好な相関を示した.CDP-choline, dexamethasone投与による検討結果では, 3. 投与後のMDSLの増加量はcontrol群(N=6) 1.16±0.20mg/100ml, CDP-choline群(N=5) 1.39±0.20mg/100, ml, dexamethasone群(N=5) 7.76±1.89mg/ml,でdexamethasone群はcontrol群(p<0.01), CDP-choline群(p<0.02)に比し有意に増加した. 4. total lecithinの増加量はcontrol群(p<0.001), CDP-choline群(p<0.001)に比して有意な増加量であつた. 5. L/S比の増加量はcontrol群0.9±0.3, CDP-choline群1.1±0.3, dexamethasone群6.5±1.0でdexamethasone群ではcontrol群(p<0.001), CDP-choline群(p<0.005)に比して有意な上昇度を示した. 6. surfactant titerはdexamethasone群,CDP-choline群ではいずれもcontrol群(p<0.05)に比して有意な増加量であつた.
