To study the nature of low back pain in competitive swimmers, especially in butterfliers, various investigations were performed on 53 swimmers. The investigations consisted of taking history, evaluation, physical examination, and X-ray examination. There was a tendency that the numbers of swimmers who experienced low back pain and who showed abnormalities on orthopedic physical examination were more in butterfliers than in non-butterfliers. In radiological survey, spondylolysis and narrowing of intervertebral disc of lumbar region were more common to butterfliers (22.0%) than non-butterfliers (0%) with statistical significance. The author considered that repeated mechanical stress to the lower lumbar spine caused by special breathing action and dolphin kick to the butterfly stroke lead to spondylolysis as a stress fracture or degeneration of intervertebral disc. Imbalance between back muscle strength and abdominal muscle one and/or increased lumbosacral angle might also have a role for the cause of the above mentioned disorders. The exercises to strengthen the muscles related to the low back are very important in the training of butterfliers.
