Valproic acid (VPA) is widely used as a relatively safe and effective anticonvulsant. However, its use in therapy has been associated with hepatotoxicity, including reversible hepatic dysfunction and irreversible hepatic failure. VPA is a simple eight-carbon branched-chain fatty acid and is metabolized to various compounds in the liver in several metabolic pathways including conjugation with glucuronic acid, β-oxidation, ω-, ω-1- and ω-2- oxidations, and γ-and δ-dehydrogenations. Some of its metabolites are considered to contribute to pharmacological actions and toxicity. Mechanisms of hepatotoxicity and anticonvulsant actions, whether mediated by VPA or its metabolites, are still little understood. Valproic acid is often used in combination with other anticonvulsants. Their possible interactions may have important clinical implications. We presented the pharmacokinetics of VPA and its metabolites and effects of other anticonvulsant drugs on metabolisms of VPA.
