When faced with a person who has severe problems with "disability" in the UK, it is often difficult to get to grips with the range of problems present. This is probably especially true when trying to read and collate reports provided by different people, but in fact it is also true for British clinicians faced with the patient and family and it is true for those responsible for planning or organizing services. One reason for the difficulty is that the problems are presented outside any recognized framework, and usually mix social problems, family problems, financial problems and "straightforward" medical problems. It is therefore a great help to use a systematic approach to analyze the situation. In order to provide background information for our discussions at this Conference in Oxford, one such approach is described here. It is often known as the World Health Organization (WHO) International Classification of Impairments, Disabilities and Handicaps (ICIDH) model, but in fact it extends that model somewhat.
