Interleukin 6 (IL-6) is a potent cytokine, the biological activities of which include the stimulation of immunoglobulin secretion, T cell activation, induction of the acute phase response, activation of megakaryocytes, and pyrogenicity. These biological activities are involved in the expression of symptoms in rheumatoid arthritis. In this study, the effects of interleukin 1 beta (IL-1β) and synovial fluid on the production of IL-6 and on the expression of IL-6 mRNA by synovial fibroblasts were tested. Cultures of fibroblast-like cells were established from synovial tissue from patients with rheumatoid arthritis (RA) and with osteoarthritis (OA). Exposure of these cells and normal skin fibroblasts to IL-1β increased IL-6 secretion. The supernatant from IL-1β stimulated RA synovial fibroblasts contained significantly higher levels of IL-6,as compared with OA synovial fibroblasts and normal skin fibroblasts. IL-6 gene expression was studied in synovial fibroblasts representative of each disease state and normal skin fibroblasts using oligonucleotide probes specific for human IL-6. IL-6 mRNA expression increased by culturing synovial fibroblasts and normal skin fibroblasts in the presence of IL-1β (5 ng/ml). Significantly higher expression of IL-6 mRNA was observed in RA synovial fibroblasts than OA synovial fibroblasts and skin fibroblasts. Next, the effects of synovial fluids obtained from patients with RA and OA on the expression of IL-6 mRNA in synovial fibroblasts were examined. RA synovial fluid markedly enhanced the expression of IL-6 mRNA in RA synovial fibroblasts, and also slightly enhanced it in OA synovial fibroblasts. By contrast, OA synovial fluid less enhanced the expression of IL-6 mRNA only in RA synovial fibroblasts. These findings indicated that the upregulation of production of IL-6 by synovial fibroblasts may contribute to the perpetuation of joint inflammation in RA.
