<原著>成犬動脈壁におけるメタリックステントによる内膜肥厚に関する形態学的検討

As a basic experiment on the use of metallic stents to treat vascular stenosis, particularly restenosis after percutaneous transluminal angioplasty, a self-expandable metallic stent was inserted via the carotid artery through a catheter into a canine aorta and morphological changes of the aortic wall were examined electronmicroscopically and immunohistochemically. Four weeks after stenting, the metallic stent was completely covered by neointima and no thrombus was found. The surface of the intima was lined by immature endothelial cells with considerable microvilli. Both edema and cellular proliferation were responsible for thickening of the neointima. Two types of cells were recognized in the edematous neointima on electronmicroscopic observation, i. e., spindle-shaped fibroblastic cells and irregular smooth muscle-like cells. All of these cells were positive for α_1-actin, whereas spindle-shaped cells were positive for both α_1-actin and desmin, indicating mesenchymal characteristics. These results indicate that phenotype changes of proliferated smooth muscle cells may occur in neointima, i.e., from the synthetic type to the mature contractile type. The possible mechanisms of such intimal changes are discussed.
